Canal Winchester Rehabilitation & Nursing discharge planning lets you focus on recovery!
While recuperating from a hospital stay, Canal Winchester guests should focus on their recovery and return home and let our staff focus on the rest. Discharge planning for our post-hospital rehab patients begins at admission, with progress clearly communicated throughout your stay. Communication is the key to a successful rehabilitation plan. A discharge planner meets with each patient and their family to help determine the patient’s needs and current support system in order to provide a safe and smooth discharge to home. The discharge planner also coordinates the patient’s interdisciplinary team of medical and therapy staff.
Medicare, Medicaid and insurance billing assistance
Insurance and billing concerns shouldn’t interfere with your care and recuperation. At Canal Winchester, we give you as many options as possible to cover your care. We are Medicare and Medicaid certified, and we accept a variety of insurance plans, private pay and long-term care insurance.
For the convenience of our short-term rehab patients, our long-term residents and their families, our business office bills Medicare, Medicaid or your private insurance on your behalf. We simply ask that you provide us with complete and accurate financial and insurance information at the time of admission. Without it, your claim could be denied and we would be required to bill you for our services. We strive to help maximize benefits, while allowing our guests, residents and families to focus on health and recovery.
